For your first visit with us, we will review all treatment you may need and make sure you understand all parts of your treatment.
Download the New Patient Registration Forms – These forms are required for all new patients or for patients returning to our office after an extended absence (over two years).
You should print the forms out then fill it in manually. Please bring the forms with you to your appointment.
Consent for Treatment of Unaccompanied Minors – In the event that you can not bring your (minor) child to his/her appointment, this form must be filled out and signed prior to the appointment.
We offer Patient Financing to our patients to finance 100% or only a portion of your treatments. Approval of credit is fast, with a possibility of tax savings on most treatments.
